Both were residents at Tattnall Healthcare Center, a nursing facility on Memorial Drive in this small south Georgia city.

The inspection was conducted on August 21, 2025, following a complaint.

The section of the admission agreement where those charges should have been spelled out had been left empty.

The name of Staff A, identified in inspection records as the Director of Facilities, appeared as active.

In each case, the vital signs documented after the fall were pulled from records made days before it happened.

The inspection, completed August 21, 2025, was triggered by a complaint.

"This past weekend we only had one housekeeper and one laundry aide for the entire building," the supervisor told inspectors.

That was the day after the facility had already concluded its investigation.

The finding came through three separate complaints filed against the facility.

Not one that staff had misread or misapplied.
