The resident, identified as Resident 3 in federal inspection records, dropped from 219 pounds in September to 173 pounds by February, a 23.29% weight loss.

The complaint involved three residents who all requested that the same two certified nursing assistants be removed from their care assignments.

The dangerous temperatures at Weakley County Rehabilitation and Nursing Center put cognitively impaired residents at immediate risk of burns.
