On the day of admission, staff recorded a Stage II pressure ulcer on the resident's coccyx, measuring one centimeter by 0.3 centimeters.

Inspectors visited the facility on January 27, 2026, and checked both of its shower rooms.

The January 29 inspection, triggered by a complaint, found that staff failed to clarify physician orders for a resident identified in records as Resident 5.

Resident 54 is a woman with cerebral palsy, severe malnutrition, and a nasogastric feeding tube that delivers nutrition directly into her stomach.

The resident, identified in inspection records only as Resident 2, said she had told staff about the gnats repeatedly.

The facility's safe resident handling policy, revised in May 2025, states plainly that two staff members must be present whenever a mechanical lift is used.

The facility houses 82 residents.

Resident #42 told his father he had no idea what he was talking about.

The resident, identified in federal inspection records only as Resident 1, had money missing from his account.

The finding came out of a January 2026 complaint inspection at Mesa Glen Care Center, a skilled nursing facility in Glendora.

What they found was a straightforward equipment problem that turned into a prolonged nutritional failure because no one was checking.
