Federal inspectors cited Greencroft Healthcare, a nursing home at 1225 Greencroft Dr in Goshen, Indiana, following an August 2025 complaint investigation.

That decision, made sometime before August 11, 2025, is what federal inspectors found when they arrived at the Chicago nursing home on a complaint inspection.

The deficiency, recorded under F0921, found that the facility failed to maintain proper cleaning of resident rooms and bathrooms.

When residents wanted to eat outside of the facility's standard schedule, suitable alternatives weren't being made available.

The August 2025 inspection at Benbrook Nursing & Rehabilitation Center, a 1,000-bed facility on McKinley Street, was triggered by a complaint.

It was one of 15 deficiencies cited during that visit.

At Glendale Place, that separation was not in place.

The inspector arrived at 9:15 a.m.

Her physician orders required continuous tube feeding.

The inspection at Autumn Lake Healthcare at Cherry Lane, conducted on August 13, 2025, turned up 12 separate deficiencies.
