Shelton Health and Rehab: Residents Go Weeks Without Showers - WA
The inspection, completed March 19, 2025, identified two residents who had gone weeks without showers despite care plans that called for bathing twice a week. In both cases, nursing supervisors reviewed the electronic health records during the inspection and found no documentation that showers had been provided.
Resident 163 was admitted to the facility in early March 2025. Her care plan, initiated March 3, called for showers twice a week, on Wednesday and Saturday evenings. On March 10, at 10:25 in the morning, she told an inspector: "I have not had a shower yet, I have been nine days, I requested one today."
The nursing assistant task records showed the shower entry for March 8 had been marked "not applicable." Nothing else appeared in the system.
When the Resident Care Manager, a registered nurse identified in the report as Staff C, pulled up her electronic health record on March 14, he said he could not find a shower documented anywhere. His expectation, he said, was for staff to follow the resident's admission orders. The Director of Nursing Services, Staff B, reviewed the same record that afternoon and reached the same conclusion. She said she could not find documentation of Resident 163 receiving a shower. Her expectation, she said, was for staff to provide showers unless a resident refused.
Resident 163 had not refused. She had asked.
The second resident, identified as Resident 14, was severely cognitively impaired and entirely dependent on staff for bathing and personal hygiene. She could not ask for a shower the way Resident 163 had. Her care plan called for showers on Mondays and Thursdays.
Her last shower, according to the facility's own shower log, was February 24, 2025. When a second Resident Care Manager, Staff D, checked both the electronic health record and the physical shower binders on March 14, she said, as far as she could tell, Resident 14 had not been bathed since February 24. That was 18 days.
The Director of Nursing reviewed the situation three days later, on March 17. She said it did not meet her expectations.
What the inspection report does not contain is any explanation for how two residents on the same bathing schedule, at the same facility, went weeks without showers while supervisors remained unaware. The Resident Care Manager for Resident 163 described his expectation as staff following admission orders. The Director of Nursing described her expectation as staff providing showers unless residents refused. Both expectations went unmet. Neither supervisor offered an account of why.
For Resident 163, the failure was documented and then invisible. A task entry marked "not applicable" on March 8 closed the record without triggering any follow-up. For Resident 14, who could not speak for herself, there was no entry at all, and no one checked until an inspector arrived.
The inspection cited the facility under Washington state regulations governing personal hygiene and bathing. CMS rated the harm level as minimal or potential for actual harm, affecting few residents.
Resident 163, moderately cognitively impaired and needing substantial to maximal assistance with bathing, had been at the facility for nine days when she told an inspector she still hadn't been clean. She said she had requested a shower that same morning.
The record does not say whether she got one.
