The admission came on August 15, 2025, the same afternoon a complaint inspection was underway.

The resident, identified in inspection records only as RI #151, lived on the facility's dementia unit.

Nobody on staff could explain how he got there, or why he had the device on in the first place.

At Chestertown Nursing and Rehab, inspectors found the process wasn't being completed as required.

Physicians were not notified and orders were not obtained.

The citation, issued August 11, falls under a category regulators use for quality of life and care failures.

The food safety citation, classified under federal tag F0812, was rated at scope and severity level F.

The finding came out of an August 11 complaint inspection at Integrity HC of Marion, a nursing home at 1301 East Deyoung Street.

The facility's menu for that Wednesday called for size-eight scoops, which hold a half-cup of food.

The citation was classified as isolated, with no documented harm to residents but with potential for more than minimal harm.

The facility was cited for failing to honor residents' right to a safe, clean, comfortable, and homelike environment.

That clinical judgment was not in dispute.
