Inspectors determined the facility fell short of that standard.

Federal health inspectors cited the facility in August 2025 for failing to maintain working call systems in resident bathrooms and bathing areas.

The resident, identified in inspection records only as Resident 16, lives at Ridgeview Health Services, Inc., a nursing home at 907 11th Street in Jasper.

The admission came on August 15, 2025, the same afternoon a complaint inspection was underway.
