Inspectors documented both failures during a complaint inspection completed August 9, 2025.

The resident, identified in inspection records only as Resident 7, was placed on a high-dose Vitamin D supplement.

The care plan, in place since July 2024, was explicit: staff were to wear appropriate PPE for any wound-related care.

Inspectors found the facility fell short.

The inspection at North Auburn Care, located at 2830 I Street Northeast, was completed April 21, 2025.

The manager said staff search the building.

The deficiency was one of 12 cited during a complaint inspection of the facility on Cherry Lane.

The citation, issued August 11, 2025, covered both routine and 24-hour emergency dental care.
