Rivers Edge Nursing and Rehab: Missing Clothing Violations - WI
Federal inspectors visited Rivers Edge on April 21, 2025, and documented what they found when they started asking questions about the missing item belonging to a resident identified in inspection records as R10.
On the morning of April 7, a surveyor sat down with the facility's Housekeeping and Laundry Manager and asked what happens when a resident reports a missing clothing item. The manager said staff search the building. If they can't find it, the facility replaces it. Asked how long they wait before telling anyone else, the manager said about two weeks. After a couple of weeks, she said, they let medical records know so a replacement can be ordered.
She had heard about R10's missing scrub top from a laundry aide only the week before the interview. That was the first she knew of it.
The surveyor noted that neither the laundry aide nor the housekeeping manager said they would document the missing item, file a grievance, or report it to the Grievance Official. Neither one mentioned those steps at all.
That afternoon, the surveyor asked the nursing home's administrator the same basic question: what do you expect staff to do when a resident says something is missing? The administrator's answer was thorough. He said he would expect staff to notify the Social Worker, search the building, ask other staff to help look, fill out a grievance, and replace the item if it couldn't be found.
What he described and what his staff actually did were not the same thing.
The gap between the administrator's expectations and his staff's behavior is the core of what inspectors documented. The administrator laid out a clear, multi-step process. His housekeeping and laundry staff described a different one entirely, one that involved waiting two weeks and quietly notifying medical records, with no grievance, no documentation, and no report to anyone designated to handle complaints.
Inspectors classified the violation as causing minimal harm or potential for actual harm and noted that few residents were affected. The deficiency falls under federal standards requiring nursing homes to have a grievance process that residents can use and that staff actually follow.
For R10, the resident whose gray scrub top started all of this, the record doesn't say whether the shirt was ever found.
