When a federal inspector sat down with a breakfast tray at Rivers Edge Nursing and Rehab on April 3, the milk registered 53.2 degrees Fahrenheit.

Windemere had to move fast to get out of it.

It had no contact information for the hospice provider.

They are also supposed to capture something less obvious: the services a resident has refused, and what the facility will do in response to that refusal.

The facility is Windemere at Westover Hills, on Christus Hills on the city's west side.

She also carried diagnoses of major depressive disorder and anxiety disorder.

That is what federal health inspectors found at Frostburg Village Rehab Center during a complaint inspection completed August 13, 2025.

The deficiency involved elopement protections for residents who cannot be trusted to stay safely inside.

Inspectors determined the facility had failed to provide and implement an adequate infection prevention and control program.

The facility had been cited for the same problem before.

The MDS Coordinator had no idea what anyone was talking about.

Without them, residents have no documented starting point for an appeal.
