That was just one item on a list that grew longer the further inspectors walked through the facility's kitchen on the morning of April 27.

One resident, out of forty at the facility, was affected.

It took inspectors less than a day on site to find it.

Inspectors visiting the facility on April 27 and 28 found that sign still hanging.

State inspectors observed it happening in real time on April 28, 2026, starting just after five in the evening.

Two of those records had problems serious enough to document.

That was not the only time someone entered his room that evening and left without helping him.

Inspectors who visited on April 30 reviewed temperature logs going back to February and found the same problem meal after meal, week after week.
