On the morning of September 10, inspectors watched a licensed practical nurse perform that dressing change.

Inspectors first observed the problem on September 7, during an early morning walkthrough of the facility's first floor.

The bag, at that moment, was on the floor next to her chair.

All of it was in the Assisted Living Kitchen that prepares food for the facility's 98 residents.

That resident, identified in inspection records as R5, was admitted to the facility with cognitive impairment and behavioral issues.

Federal inspectors observed the unlocked cart at 12:07 p.m.

Somewhere in that cycle, the medication stopped.

The Enhanced Barrier Precautions sign was posted directly on the resident's door.

The facility serves 76 residents.

That admission came from the facility's own director of nursing during a September complaint inspection.

The nurse at the center of the finding is identified in the inspection report only as RN-D.

The form was completed by the resident's responsible party, the person legally authorized to make medical decisions on the resident's behalf.
