The notices arrived at Maple Grove Health and Rehabilitation Center on August 22, 2025, and again on September 29, 2025.

The driver transported the resident without properly fastening the restraint.

The dosage was transcribed incorrectly upon admission for a resident identified in inspection records as Resident R8.

The staffing posting violation was one of 11 deficiencies cited during the complaint inspection, conducted on November 20.

The resident, identified in inspection records as Resident 78, needed two-person assistance for bed mobility, bed baths, and showers.

Inspectors found the facility's fall policy contained no direction for staff on how to report falls once they occurred.
