Good Samaritan Society - Waukon is part of one of the largest nonprofit nursing home chains in the country.

The inspection at Wellsprings Care Center, completed September 11, 2025, documented at least six falls by a single resident between late June and late August.

From July through at least the date inspectors arrived, no qualified full-time social worker was on staff.

The violation falls under a category regulators call Quality of Life and Care Deficiencies.

York Manor Nursing Home was cited for 18 separate deficiencies during a complaint inspection conducted on September 15, 2025.

That is what inspectors found when they investigated a complaint at Autumn Lake Healthcare at Loch Raven, an 8720 Emge Road nursing facility in Baltimore.

The violation was tagged at a level of minimal harm or potential for actual harm.

But inspectors concluded the gap between what was ordered and what was delivered carried real potential for harm to go beyond the minimal.

The September 2025 complaint inspection did not describe what the nurses did wrong, or what happened to the resident.
