The name of Staff A, identified in inspection records as the Director of Facilities, appeared as active.

In each case, the vital signs documented after the fall were pulled from records made days before it happened.

"This past weekend we only had one housekeeper and one laundry aide for the entire building," the supervisor told inspectors.

That was the day after the facility had already concluded its investigation.

The finding came through three separate complaints filed against the facility.

Not one that staff had misread or misapplied.

The kitchen was unclean and unsanitary, inspectors noted, and the droppings were described as substantial in amount.
