She wasn't wrong to be confused.

That is what inspectors found when they arrived at Aliya of Glenwood on August 22, 2025.

The observation happened at 3:10 p.m.

Between batches, she washed the bowl with detergent and rinsed it with water.

Federal inspectors documented the incident during a complaint inspection completed August 21, 2025.

In the four days between those two dates, the aide continued working with other residents at Mountainside Skilled Nursing and Rehab.

Three of those four items belonged there.

The colleague, V6, had prepared for a mechanical lift transfer.

The dishes went into the cabinet.

The resident, identified in inspection records only as R5, had wounds documented across at least four separate body sites by mid-July 2025.

The incident had happened more than two months earlier.

That is what inspectors found at Providence Living Center, a 175-bed facility at 1112 SE Republican Avenue, during an August 25 complaint inspection.
