Willow Brook Christian Home: Lift Injury Splits Resident's Head - OH

Two certified nursing assistants had just finished transferring the resident from her bed to her wheelchair. CNA #68 left the room. CNA #62 stayed behind to move the mechanical lift away from the chair. As she did, the metal edge of the lift's cradle swung into the resident's head. The resident began bleeding immediately.
CNA #62 told inspectors she grabbed a towel and pressed it to the wound, then called for a nurse.
The resident was transported out for treatment. X-rays were taken of her right forearm, right hip with pelvis, right humerus, and right tibia fibula. The laceration on her head was closed with four staples. She also received a diphtheria, tetanus, and pertussis immunization.
The Assistant Director of Nursing confirmed to inspectors what CNA #62 had described: the resident was struck in the head by the cradle of the mechanical lift while the aide was moving it away from the wheelchair.
What inspectors found when they looked at the lift tells the rest of the story. The manufacturer's own materials, a pamphlet titled "Electric Patient Lift," listed a swivel bar protective covering as a standard component of the lift's main assembly. The covering was meant to be on the lift from the start. It wasn't.
By the time inspectors observed the lift on August 29, padding had been placed on the swing arm. The ADON told inspectors that adding the padding, as described in the manufacturer's instructions, was the intervention the facility put in place after the accident.
CNA #62 told inspectors she had her own plan: she said she would make sure two staff members were always present until the mechanical lift was moved away from the resident. That the lift had struck someone in the head was, in her telling, a problem of proximity and staffing. That the lift was missing a protective cover the manufacturer included as a required component went unmentioned in her account.
The inspection was conducted on August 29, 2025, and the citation was assigned under complaint number 2566262.
The resident left the facility with staples in her head and a fresh tetanus shot. The padding went on the lift after.
