The citation, issued under the nutrition and dietary deficiency category, covered the handling and disposal of garbage and refuse at the facility.

The same thing happened the next morning.

The resident, identified in inspection records only as Resident #59, had a care conference in October 2024.

The finding emerged during a federal inspection of North Auburn Care, a nursing facility at 2830 I Street Northeast, completed April 21, 2025.

The resident who was assaulted is identified in inspection records only as Resident #80.

According to the inspection report, she communicated through gestures, pointing to her belly, her throat, her lower back, opening her mouth to signal distress.

The resident, identified in inspection records only as Resident 169, came to Grand Trace on March 22, 2025, with diagnoses of neck pain and pain.

That finding came out of a complaint inspection completed May 22, 2026, at Avalon Care Center at 1930 Kamehameha IV Road in Honolulu.

They came back the next morning.
