Federal health inspectors arrived at the Los Angeles facility on November 6, 2025, responding to a complaint.

These aren't abstract procedural requirements.

The citation, classified under F0689 with a harm level of "Actual harm," appeared in a complaint inspection completed October 23, 2025.

The pain management failure was cited under a federal regulatory category covering quality of life and care.

One of them, identified in inspection records as NA-E, told her colleague she didn't think the full body lift was correct for this resident.

Together, a GNA and a member of the housekeeping staff lifted Resident 13 back into bed.

The concern is that residents were being chemically controlled.

Inspectors did note the potential for more than minimal harm to residents who did not receive the services.

That gap, between what the resident said and what the medical team knew, sat at the center of a complaint inspection completed in November 2025.

She said it wasn't the first time.

The incident at the center of it dated to a Saturday, September 13, five weeks earlier.

The fall came to light during a complaint inspection completed October 17, 2025.
