That was one moment during a single lunch service on May 18.

The resident, identified in inspection records only as R1, lives at Ascension Nazarethville Place, a nursing facility at 300 North River Road in Des Plaines.

What replaced it at Grove at Kirkwood was a lavender-scented all-purpose cleaner called Medorra Limpreza.

The director of nursing, when interviewed by inspectors, did not dispute what had happened.

The citation, issued April 24, found that the facility failed to provide safe and appropriate pain management to a resident who required those services.

State health officials received no written report.

The inspection, completed April 24, 2026, centered on Resident 33.

That notification is not optional.

Repeated falls in her history, muscle weakness, an unsteady gait, and a body that requires moderate staff assistance just to get on and off a toilet.

The resident, identified in inspection records only as Resident #59, was admitted on May 14, 2026.

Staff were also required to initial the medication administration record immediately after giving each medication and before moving on to the next one.

When a resident doesn't know what medications they're on, what a diagnosis means, or what a nurse is about to do, they cannot agree or refuse.
