State inspectors who visited the facility on September 11, 2025, found the problem ran deeper than one missed order. The facility had no standing orders at all governing how staff should manage tube feedings. The medical director had not written them out when writing orders for tube-fed residents. Nobody had.
When inspectors asked how the facility tracked whether a resident actually received their full feeding, the administrator and director of nursing explained that the facility did not record the total amount of enteral feeding a resident receives. Staff were expected to notify the physician if a resident hadn't tolerated or received a full feeding, and to document it in progress notes. Staff were expected to know how to manage tube feedings on their own.
The physician, for their part, had assumed the facility's standing orders were being entered automatically. They were not.
A resident had been on a Fibersource HN feeding regimen. The pump wasn't delivering the full amount. Weight was dropping. None of it had surfaced to the people responsible for overseeing the facility's care.
Inspectors cited the deficiency at a level of minimal harm or potential for actual harm, with few residents affected. The finding was part of a complaint inspection.
What the administrator and director of nursing said they expected from nursing staff, and what nursing staff had actually done for at least one resident losing weight on an incomplete feeding regimen, were not the same thing.
