Aspire Senior Living Webb City: Shower Neglect Findings - MO
Nobody told him to do it that way. Nobody caught it either, not for a while.
The arrangement unraveled after residents started complaining to the facility's Social Services Director that they weren't getting showers. The SSD pulled a shower report and brought the findings to the Director of Nursing. A few days before inspectors arrived on August 13, 2025, the picture had become clear enough to act on, but the problem had been running long enough that neither the DON nor the administrator knew how long it had been going on or how many showers had been missed.
The Director of Nursing told inspectors she had not known the shower aide was logging refusals when he simply ran out of time. The administrator said the same thing: he or she was not aware staff were failing to assist residents with showers as scheduled.
What the inspection found was a system that had no real backup. When the shower aide couldn't finish his list, whether because he ran out of time or was pulled to work the floor, the residents on the remaining list simply didn't get bathed. The aide wasn't handing off the unfinished list to the charge nurse, which was what the DON said should have happened. The charge nurse could have assigned other aides to finish the showers, or at minimum documented an actual reason why a shower wasn't given. Instead, the aide wrote down refusals that weren't refusals, and the records looked clean.
Floor aides, for their part, didn't always have time to pick up the slack. The facility required that every resident receive at least supervision for safety during showers, which meant the showers couldn't simply be handed off informally. Someone with the time and the assignment had to be there.
The administrator told inspectors that all residents should be offered at least two showers per week, unless a resident personally preferred a different schedule.
That standard, two showers a week as a floor, is not a high bar. It is the kind of baseline that exists precisely because bathing is not optional for people who cannot fully care for themselves, people who may not be able to speak up loudly enough, or consistently enough, to be heard. The residents at Aspire who did speak up went to the Social Services Director. That conversation is what started the investigation. The ones who didn't speak up, or couldn't, had their silence recorded as a choice.
The inspection was conducted in response to a complaint. The violation was cited at a level of minimal harm or potential for actual harm, and inspectors noted that some residents were affected.
What the report does not say is how long the false documentation had been accumulating, or how many residents went without showers they were owed while the records showed something else entirely.
