He told inspectors he had only learned the full story about the situation that morning, after they arrived and started asking questions. "I would never stop V13 from coming to visit," he said. "Unless there is an order of protection, they cannot restrict a visitor. It's the resident's right to have visitors."
The resident's own admission record, dated the day of the inspection, listed V13 as his significant other. His care plan contained no mention of any restricted or limited visitation. The facility could not produce a single document with legal authority to keep her out.
The facility's own visitation policy, reviewed just three months earlier in May 2025, states that residents have the right to receive visitors and that any restriction of visitation must be documented and reported to the administrator. None of that happened.
V13 kept calling. The kiosk kept reading "Access denied." And the man inside, whose admission paperwork named her as his girlfriend, had not seen her in at least two months.
