Aperion Care Forest Park: Ceiling Collapse Neglect - IL
That is what inspectors found at Aperion Care Forest Park during a complaint inspection on May 19, 2026.
The room was shared by two residents. One of them, identified in inspection records as R1, is blind, non-ambulatory, and has a history of falling. She also has morbid obesity and major depressive disorder. The other resident, R2, has a history of congestive heart failure, spinal stenosis, and anxiety. Both women were living beneath a ceiling with six tiles gone, open to whatever was above them, for somewhere beyond a month before a federal inspector walked in.
R1 told the inspector her sister had noticed the tiles were still missing. "My sister said they have not replaced the ceiling tiles, they've been down for a while," she said. R2 was more specific. "The ceiling tiles have been missing for over a month," she said. "Water was coming down also."
The maintenance supervisor, identified in the report as V4, told the inspector two days later that he didn't know exactly how long the tiles had been down. His explanation: he was waiting for the roof to be repaired first. "I was waiting for the roof to be repaired," he said. "I will replace the tiles today."
The administrator, identified as V1, confirmed the sequence. The roof had needed replacing, she said, and now that it was done, the tiles could go back in.
What neither explanation addressed was the month or more that passed in the meantime, while two medically fragile women lived in that room.
The facility was asked to produce a policy governing room maintenance and environmental upkeep. It could not produce one.
CMS rated the level of harm as minimal, the lowest tier on the federal scale. That classification reflects regulatory judgment about the severity of documented harm, not necessarily the experience of the people living in the room. R1 cannot see the gap in the ceiling above her. She can hear what comes through it. R2, who has anxiety and a damaged spine, spent more than a month knowing water had already fallen into the space where she sleeps.
The inspection covered two residents and found the same problem in both of their lives. That was the full scope of the environmental review. Whether other rooms in the facility had similar issues, the report does not say.
Aperion Care operates a chain of facilities across Illinois. The Forest Park location sits in a western suburb of Chicago. The complaint inspection that produced this finding was completed May 22, 2026.
What the maintenance supervisor promised to do that afternoon, after the inspector had already seen the room and written down what was there, was the same thing that could have been done the day the tiles came down.
R2 said the water came down too. Nobody in the report said when that stopped.

Data source: This article is based on inspection data downloaded directly from the Centers for Medicare & Medicaid Services (CMS) via Medicare.gov. CMS releases inspection reports in bulk; we publish the findings as documented by state surveyors in the official Form CMS-2567 Statement of Deficiencies.

Corrections may have occurred: Inspection reports reflect conditions observed on the date of the survey. Facilities may have implemented corrections, staffing changes, additional training, or other remediation since the report was issued.
