A staff witness, identified in the inspection report as V45, told investigators it happened about two weeks before the November 17 inspection. V45 was working the 300 hall that night. The two agency CNAs left at approximately 8:40 PM. They did not return until 10 PM, when the shift ended and they could collect their hours.
For that stretch of time, 74 residents were in the building with fewer hands than the facility's own standard requires.
The administrator, identified as V1, told inspectors on the afternoon of November 17 that agency staff are brought in to cover call-ins, and that the facility consistently meets minimum staffing ratios on night shift, which V1 said requires three CNAs. V1 did not dispute that agency workers had left. The administrator's position was that the numbers still added up.
V45 did not appear to share that confidence.
The facility's own staffing policy, dated December 2024, states that actual hours worked per position will be posted daily at the start of each shift. Whether that posting reflected what actually happened the night the two agency CNAs walked out is not addressed in the inspection report.
Inspectors tagged the deficiency under F0725, which covers sufficiency of nursing staff, and assessed the level of harm as minimal harm or potential for actual harm, with many residents affected.
