Documentation: A copy of the discharge summary will be provided to the resident and or the resident's family member upon discharge when return is not anticipated.

The discharge summary will include the following information.

Recapitulation of the resident stay, including, but not limited to, diagnosis, course of illness, treatment, pertinent labs, radiology and other consultation reports. A copy of the discharge summary and discharge care plan will be maintained in the residence medical record.
