During an observation on 3/30/26 at 3:30 p.m., revealed the Clindamycin topical gel was available with a dispense date of 3/23/26 on the label.

During an interview with the Director of Nursing on 3/30/26, at 3:30 p.m. confirmed that the facility failed to follow a physician order for medication administration for Resident R1.

(See instructions.) Except for nursing homes, the findings stated above are disclosable 90 days following the date of survey whether or not a plan of correction is provided.

For nursing homes, the above findings and plans of correction are disclosable 14 days following the date these documents are made available to the facility. If deficiencies are cited, an approved plan of correction is requisite to continued program participation.
