That is what inspectors found at Valley View Skilled Nursing and Rehabilitation following a complaint investigation completed December 23, 2025.

The records that did exist told a fragmented story.

Four days later, a resident at Bria of Elmwood Park was found unresponsive.

Nothing went into the medical records of either resident involved.

It called for soft tacos, refried beans, tortilla soup, and a brownie for residents on a regular diet.

The complaint inspection, assigned the highest level of harm short of immediate jeopardy, found that the transfer caused real injury to a resident.

They wanted their life prolonged if feasible.

That finding was at the center of a December complaint inspection at ARC at Cincinnati, a nursing home at 4001 Rosslyn Drive.

The deficiency fell under what inspectors classify as a scope and severity level D, meaning the lapse was isolated and no resident suffered documented harm.

That resident had liver disease severe enough to cause fluid buildup, a condition called ascites.
