His September cognitive assessment showed a score of 8 out of 15, indicating moderately impaired mental function.

The specialist prescribed gabapentin, a seizure medication commonly used to treat nerve pain, at 100 milligrams daily.

The facility never issued the mandatory written discharge notice to the resident, their representative, or the state long-term care ombudsman.

The first incident occurred in October 2024.

The smell greeted surveyors on October 27 during their initial entrance and tours of the facility.
