On September 29, 2025, roughly 38 of the facility's 76 residents went without coffee at breakfast.

The resident, identified in inspection records only as R4, was found that afternoon wearing green heel boots on both feet.

The nurse had worked at the facility since April 1994.

Emergency Medical Services picked up Resident R223 at approximately 10:13 p.m.

The violation was tagged under F0689, the federal standard governing protection from accident hazards and supervision.

The inspection, completed November 26, 2025, was triggered by a complaint.

The tub contained piles of dirty clothing.

A progress note from September 19, 2025, recorded that R1 was on a civil commitment.

When inspectors arrived on November 24, 2025, they found the entire 100-hall had been without hot water since before summer.

The answer at Bernard Care Center, a 126-bed facility on West Pine Boulevard, was not much.

That gap matters because catheter care, done wrong, is one of the more direct routes to infection for nursing home residents.

The findings came from a complaint inspection conducted December 1, 2025.
