That's what inspectors found when they walked through the kitchen at Bostick Nursing Center on the morning of September 22, 2025.

He had just been lowered to the floor of a shower room at Monterey Park Rehabilitation & Health Care Center after a fall.

The problem was straightforward and the facility knew about it.

The resident, identified in inspection records as R3, had no care plan warning that he was at risk for skin breakdown before May 19, 2025.

The people it houses are not simply elderly.

Not the nurses who came to her room to administer care.

The resident, identified only as Resident R1, has lived at the facility since December 2021.

The facility's own licensed practical nurse and interim director of nursing confirmed it.

What happened after the attack took weeks to sort out, and some of it never got documented at all.

The unit had been pulled from the wall and was dangling.
