Inspectors declared an Immediate Jeopardy at The Lev at Town Park, an 8820 Town Park Drive nursing and rehabilitation facility, on November 13, 2025.

Federal inspectors were standing in the kitchen when it happened.

Immediate Jeopardy is not a warning.

The left had a small skin tear on top.

They searched inside the building and outside.

That's what inspectors found when they visited the facility on November 17, 2025, following a complaint.

The resident's physician did not find out any of this had happened until three days later.

Inspectors rated the harm as minimal or potential, and noted few residents were affected.

At the hospital, she received another milligram of Narcan.

That explanation didn't hold up once inspectors pulled the paperwork.

The resident, identified in inspection records as R2, was admitted to Beaver Dam Health Care Center with sepsis.

The aide who frightened her, identified in inspection records only as CNA E, was fired six weeks later.
