At some point before inspectors arrived, the tube had become dislodged.

The policy was less than three months old when inspectors found the facility wasn't following it.

That is what inspectors found when they visited Wisconsin Rapids Health Services on October 6, 2025.

The incident surfaced in a federal complaint inspection completed September 30, 2025, and the details came from the aide himself.

The staff members included four certified nursing assistants, two licensed practical nurses, and a food service assistant.

The nurse, identified in the inspection report as V8, was working his first shift at the facility.

The finding at Pleasant Meadows Senior Living, at 400 West Washington, affected a small number of residents.

Stage 4 is the most severe classification, reaching through skin and tissue to muscle or bone.
