Inspectors visited the facility on March 17, 2025, and found Resident 75 lying in bed with the tobacco cans in plain sight at 10:50 in the morning.

The inspection report does not give the resident a name.

The resident, identified in inspection records only as Resident 8, had been diagnosed with hemiplegia, diabetes, and hypertensive heart disease.

Diabetes complicates wound healing significantly.

The inspection at The Hills Post Acute, a skilled nursing facility at 1800 Old Tustin Avenue, was conducted on March 24, 2025.

What they found didn't match what the paperwork said.

The facility has four registered nurses on staff.

The resident, identified in inspection records only as Resident #50, had a physician's order for oxygen that ran through March 10, 2025.

The physician's order was not ambiguous.

That detail emerged during a March 27 inspection interview that also surfaced something more troubling: the facility had no ethics or compliance policy.

The medication was Santyl collagenase ointment, a topical enzyme treatment used to clean and manage wounds.

That was the finding federal inspectors documented after a May 2026 visit to the facility at 7660 Wyngate St.
