The incident occurred on August 5th when Unit Manager #379 removed a power strip from Resident #15's room at Landings of Westerville Health and Rehab.

The resident arrived at the facility on August 6, 2025, but staff failed to detect the wound during the required admission skin assessment.

Two residents experienced multiple week-long gaps in bathing despite facility schedules calling for showers three times per week.

Their cognitive assessment showed moderately impaired mental function, scoring 9 out of 15 on a standard test.

Both residents had intact mental abilities and could understand their life-sustaining equipment wasn't working.

The facility houses 80 residents total.

The immediate jeopardy period ran from July 20 through July 21, 2025, according to the September inspection report.

Federal inspectors found Resident #123 alert and cognitively intact during their August complaint investigation.

The September 3, 2025 inspection focused on allegations of physical abuse affecting multiple residents at the facility.
