The finding came out of a complaint inspection completed August 29, 2025.

The man, identified in inspection records only as Resident R2, is in his later years and has lived at the facility since March 1, 2024.

That gap sits at the center of a deficiency citation that touches on some of the most fundamental daily care a nursing home provides.

By early afternoon, he was hard to wake up.

The September 2025 complaint inspection at the facility, located at 5608 SW 9th Street, documented what staff described as an ongoing and unresolved problem.

"They are on all the time," the aide, identified in the inspection report as V10, said during a visit on August 26.

The numbers from the main kitchen tell part of the story.

The incident involved a man identified in inspection records only as R1 and a maintenance employee identified as Maintenance U.

What they found in the medical records was a pattern of wounds discovered, noted, and then largely ignored.

He had been hired through an employment agency, which provided documentation showing an active Ohio Nursing Home Administrator license.

The aide, identified in inspection records as Employee E14, sent the email on August 23, 2025, at 5:53 in the evening.

She was dependent on staff for every activity of daily living.
