She was afraid that if she reported what she saw, the aide, identified as CNA19, might retaliate in a way that cost her a place to live.

That is what inspectors found at Achieve Rehab and Nursing Facility on 170 Lake Street when they visited on May 22, 2026.

The deficiency was cited under New York Code Rules and Regulations 415.12(a)(3), which covers personal hygiene and grooming.

Resident 7 was first admitted to the facility in November 2023.

The resident at the center of the December incident, identified in inspection records as Resident 461, had a history staff described openly.

Inspectors visited the facility on May 19 and 20, 2026, as part of a complaint investigation.

The staff member, identified in inspection records as AS10, received on-the-job training as a caregiver.

The resident, identified in inspection records only as R12, had a good relationship with the nursing assistant, identified as CNA19.

The facility sits at 6025 North Assembly, a residential stretch of northwest Spokane.

The inspection, completed May 22, 2026, examined a complaint about care provided to Resident #153 following an unwitnessed fall on August 22, 2024.

The warning involved a resident admitted in March 2026 with chronic obstructive pulmonary disease and depression.
