Federal inspectors watched the procedure unfold at 11:41 p.m.

Inspectors with the Centers for Medicare and Medicaid Services visited the facility at 2400 Quaker Ave on March 27, 2026.

The incident at North Pointe Care Center, a skilled nursing facility at 500 Jessie Avenue in Sacramento, took place on February 6, 2025.

A cook on the morning shift had told him they were out of breakfast meat.

Call lights are the most basic tool a nursing home resident has.

The resident, identified in inspection records only as R10, had transferred to Medilodge on September 26, 2025, from an assisted living home.

The residents had been placed there, in part, because they were at risk of falling.

The resident had a fractured shoulder.

The answer, from the facility's own Assistant Director of Nursing, was nothing.

The man, identified in federal inspection records only as Resident 70, is a patient at Hawthorne Healthcare & Wellness Centre, LP, on South Grevillea Avenue.

Those duties were not complicated or obscure.

That nursing assistant was the son of the facility's administrator.
