When an inspector observed the bathroom sink on May 27, 2026, standing water sat in the basin, no stopper in place.

That was the scene inspectors documented on May 28, 2026, during a complaint inspection at the facility on Fred Rogers Drive.

Inspectors visited Kadima Rehabilitation & Nursing at Latrobe on May 28, 2026, responding to a complaint.

None of them could say why this one didn't.

Specifically, oxygen saturation levels were not being measured and documented every day for those residents.
