An inspector found the cart unlocked at 9:12 in the morning on April 23, 2026.

The resident, identified in inspection records only as Resident 3, had a physician's order for albuterol nebulization every four hours.

That was one of two abuse reporting failures federal inspectors documented at the facility following a complaint investigation completed January 29, 2026.

Neither incident reached Illinois regulators for nearly two weeks.
