Parsons Hill Rehab: Discharge Notice Failures - MA
The inspection, completed August 19, 2025, was triggered by a complaint. What investigators found was a routine administrative process that had quietly collapsed, affecting residents who were already among the facility's most vulnerable: people admitted for serious acute conditions, stabilized, and sent home without the paperwork that exists specifically to protect them.
Resident 4 was admitted in July 2025 with acute and subacute infective endocarditis and pneumonia, two serious conditions that together can be life-threatening. On admission, he scored a 15 on the Brief Interview for Mental Status, the highest possible score, meaning he was fully alert and oriented. By July 30, a nursing progress note recorded that he had been discharged to his family's home.
His medical record contained no Notice of Intent to Discharge. No copy had been sent to the Office of the State Long-Term Care Ombudsman. There was no documentation that either step had been attempted.
The ombudsman program exists to give residents and their advocates an independent check on discharge decisions. A notice requirement is not a formality. It is the mechanism by which someone outside the facility's chain of command learns that a resident is being sent somewhere, and can intervene if the discharge is unsafe or premature. Skipping the notice doesn't just create a paperwork gap. It removes the ombudsman from the picture entirely.
When inspectors sat down with the Director of Social Services at 1:39 in the afternoon on August 19, she did not dispute what the records showed. She told them she had not been issuing the Notices of Intent to Discharge to short-term stay residents. She had not been notifying the Office of the State Long-Term Care Ombudsman of those discharges either. The inspection report does not indicate how long this had been going on, or how many residents were affected beyond the two identified in the report.
Resident 2 was the other case documented. A notice had been sent to the ombudsman in that instance, which is how the inspection report references it, though the details of Resident 2's circumstances are not fully described in the findings.
Two and a half hours after the social services interview, at 4:10 in the afternoon, inspectors spoke with the administrator. The administrator said they were unaware that the facility was required to issue Notices of Intent to Discharge to short-term stay residents at all, or that the ombudsman needed to be notified.
That answer is notable for what it reveals about oversight inside the building. The social services director knew the process well enough to describe exactly what she had not been doing. The administrator, by their own account, did not know the process existed. Between them, a gap had opened that no one caught.
The violation was cited at a harm level of minimal harm or potential for actual harm, and inspectors noted that some residents were affected. That language reflects the regulatory classification, not necessarily the experience of any individual resident who was discharged without the notice they were owed.
For Resident 4, a man or woman who had survived endocarditis and pneumonia and gone home to family, the practical consequence of the missing paperwork may never be fully known. The ombudsman who was never notified could not have reviewed a discharge they were never told about. Whether the discharge was appropriate, whether the family was prepared, whether any concerns went unvoiced because no outside advocate was ever in the loop, none of that appears in the inspection record.
What does appear is a facility where the person responsible for issuing a protective notice had stopped doing it, and the person responsible for running the facility didn't know it was supposed to happen at all.
