place to assist with the weight loss. During an interview on 10/30/25 at 12:08 P.M., Physician D said the resident has been back and forth to the hospital and can be non-compliant with care. The resident eats well but will refuse meals and has an unstable blood sugar (level of sugar in the bloodstream). The facility should have identified the weight loss and put interventions in place and the RD should have been notified.
During an interview on 10/30/25 at 8:52 A.M., the Regional Nurse Consultant (RNC) said he/she was not familiar with the resident. The facility should have looked at the resident's weights during the stand-up morning meeting and put interventions into place to prevent the weight loss, which included contacting the RD and the resident's physician. The Administrator was present at that time and agreed with the RNC.
During an interview on 11/6/25 at 10:26 A.M., the DON said he/she has only been with the facility for about four days. The DON said the charge nurse and the unit manager should be reviewing the resident's hospital records for orders and any recommendations. The DON said the following day he/she or the ADON will
review the hospital records as a follow up. The charge nurse should be contacting the resident's physician and if necessary the RD. The DON said the meal consumptions should be documented in the EHR by the charge nurse. The Administrator was present and agrees with the DON. 2648439
