Develop a post-discharge plan with the resident and family for the resident's care after leaving the nursing home.

Submit a timely, acceptable plan for facility closure, including notification of the appropriate entities and ensuring residents are transferred in a safe and orderly manner.

Have policies and procedures ensuring the administrator's responsibilities for facility closure are completed successfully.
