Per V11 (Pest Control Company) service report, dated 12/11/2025, the room that was seen full of plastic bags and clutter needs to be treated for bed bugs. Per report, dead bedbugs and live bedbugs were seen. Rooms that were occupied by residents Resident R3, Resident R4, Resident R5, Resident R5, Resident R7, Resident R8, Resident R9 and Resident R10, were included on pest control report related to bedbugs. Change of rooms related to bedbugs' concern: Resident R4 and Resident R5 were relocated on 09/10/2025.Resident R6, Resident R7 and Resident R8 were relocated on 12/11/2025. Resident R9 and Resident R10 were relocated on 12/10/2025. Pest Control Policy dated 09/01/2022:The Environmental Services Director will be responsible for coordinating the facility pest control. The pest control program will be conducted on a regular and as needed basis. Employees are instructed to promptly report all observation of pests to their department heads. The facility shall be kept in such condition and cleaning procedures used to prevent the harborage or feeding of insects and rodents.
