Potential for 2. Medical record review for Resident 438 was initiated on 1/29/24. Resident 438 was admitted to the facility Some Review of Resident 438's Order Summary Report showed a physician's order dated 1/23/25, for continuous affected

Review of Resident 438's Plan of Care failed to show Resident 348 was receiving the supplemental oxygen.

On 1/29/25 at 0904 hours, Resident 438 was observed lying in bed with the supplemental oxygen being administered at 3 liters per minute via nasal cannula.

On 1/30/25 at 1227 hours, an interview and concurrent medical record review was conducted with the IP.

The IP stated the oxygen administration should be documented in the resident's plan of care.

The IP verified and stated Resident 438's care plan for oxygen use should have been initated.
