A review of the facility's policy and procedure (P&P) titled, Care Plans, Comprehensive Person - Centered, revised on 12/2016, indicated; a) a comprehensive care plan that includes measurable objectives and timetables to meet the resident's physical and functional needs is developed and implemented for each resident, b) comprehensive, person centered care plan will described services that are to be furnished to attain or maintain highest practicable physical, mental wellbeing, and include resident stated goals upon admission and desired outcomes.
