Federal inspectors visiting the Marienville facility on May 29, 2026, found that staff had discontinued the wanderguard on Resident R85's right wrist in January 2024, on a physician's order. The resident, admitted in September 2022 with diagnoses including schizoaffective disorder and COPD, had been flagged as an elopement risk since May 2023. The intervention calling for bracelet checks every shift was never removed from the care plan. Four separate assessments spanning July 2025 through April 2026 all recorded that no wander or elopement alarm was in use. The care plan said otherwise.
A Licensed Practical Nurse Assessment Coordinator confirmed during an interview that the wanderguard had been discontinued and the care plan was never updated. "It should have been," she said.
The second resident, R6, had been a patient at the facility since December 2012, with a history of kidney obstruction, diabetes, and high blood pressure. A progress note from February 2026 recorded that the urinary catheter had been removed. The care plan, last dated December 2025, still listed interventions to change the catheter, flush it, check tubing for kinks each shift, and keep it draining continuously. A May 2026 assessment showed no catheter in place. No current physician order for one existed either.
The facility's administrator confirmed at the same interview that the genitourinary care plan had not been reviewed or revised to reflect that the catheter was gone.
Inspectors cited two of 26 residents reviewed and classified the harm level as minimal. The facility's own care plan policy, dated May 2026, states that care plans are revised as residents' conditions change.
For Resident R85, the condition changed in January 2024. The plan did not.
