Nobody called the Plant Operations and Maintenance Director Consultant, Staff D, that night. Staff D told inspectors they didn't find out the call light system was down until 10:47 AM on May 14. "If they had been notified on 05/13/2026 that the call light system was not working," Staff D said, "they would have sent someone out and/or came themselves at that time." Staff D called it an urgent matter and said the expectation was immediate notification for any resident safety issue.
The assistant director of nursing, Staff A, saw a message on the staff phone app at 3:24 PM on May 13 that read "Cove lights not working." She told inspectors she did not investigate how many call lights were out, did not confirm residents still had a way to alert staff, and did not ensure staff knew how to proceed.
The administrator, Staff G, learned the call light system had been down since the previous day by reading a grievance pulled from the grievance box that morning. They had not been told by anyone on staff.
"If they knew," Staff G said, "they would have contacted Staff D for immediate repair."
The inspection was conducted May 14 following a complaint. Inspectors cited the failure as causing minimal harm or potential for actual harm to some residents, the people who spent a night in rooms where pressing a button would summon no one.
