Integrity HC of Marion: Environment Safety Violations - IL
Staff acknowledged the lapse. "There was a miscommunication," the facility told inspectors. "One person thought another person had cleaned the room when actually no one had cleaned it prior to" the resident's entrance. They called it an honest mistake and said there should have been better communication between housekeeping staff.
The facility's own deep cleaning checklist spells out what the job requires. Privacy curtains come down. Beds get stripped. The mattress is wiped front and back. So is the bed frame, the control panel, the nightstands inside and out, the window seal, the over-bed table top to bottom. Staff are supposed to move the nightstands and clean behind them, move the bed and clean where it sat, wipe down the walls, and move anything else that can be moved to clean beneath it. If the room has a bathroom, it gets deep cleaned. Trash cans, inside and out. The air conditioner filter and vents. Then the curtains go back up, the bed gets made, and everything returns to its place.
None of that had happened before the resident walked in.
The checklist the facility provided to inspectors carried no date.
Inspectors rated the violation at the lowest level of harm, affecting few residents. The resident involved was identified only as R2.
What the inspection record does not show is whether anyone checked the room before the resident was placed there, or whether the communication breakdown was a single failure or part of a pattern. The checklist exists. The expectation was clear. The room went uncleaned anyway.

Data source: This article is based on inspection data downloaded directly from the Centers for Medicare & Medicaid Services (CMS) via Medicare.gov. CMS releases inspection reports in bulk; we publish the findings as documented by state surveyors in the official Form CMS-2567 Statement of Deficiencies.
Plan of correction: The CMS report we receive does not include the facility's plan of correction. Facilities submit plans of correction separately to state survey agencies and those responses may not be reflected in CMS data at the time of publication. The absence of a plan of correction in our data does not mean one was not filed. Readers who want information about corrective steps taken are encouraged to contact the facility directly or their state survey agency.
Corrections may have occurred: Inspection reports reflect conditions observed on the date of the survey. Facilities may have implemented corrections, staffing changes, additional training, or other remediation since the report was issued. We report what CMS provides and encourage readers to seek current information from the facility.
Editorial process: Inspection findings are extracted from CMS source documents and synthesized using AI, reviewed for factual accuracy against the original report by our editorial team.
