Housekeeper F said no one had told him or her how much of the new cleaner to use. Without the dispensing hookup the facility used to have, he or she poured some into the cap and tipped it into the mop bucket. That bottle, he or she noted, was the last one they had.
Two days later, Housekeeper T was doing the same thing, covering the bottom of the bucket and mopping floors with the same lavender cleaner. The old hookup system, which measured and dispensed cleaner automatically, was gone.
Inspectors checked. The EPA's certified products database returned no result for Medorra Limpreza All Purpose Cleaner Lavender scent. The product's own website produced a critical error on every visit, across four separate attempts between January 28 and February 6. The cleaner could not be verified as hospital-grade or EPA-registered by any available means.
The facility's own cleaning policy required an EPA-registered hospital disinfectant for resident rooms and floors. The facility had no Housekeeping Director. The Regional Nurse Consultant confirmed there had been no training for staff on chemical dilution.
A central supply worker who had been at the building for years said the problem started September 1, 2025, when new management took over. By October, she had been handed medical records and accounts payable on top of her existing duties. Supplies, she said, went from reliable to drastically limited almost immediately. "Before the management change, there was never a problem with supplies," she told inspectors. "When they took over, there was a budget issue."
The Administrator told inspectors there were extra supplies in the basement and that he or she had told staff to find them. He or she said housekeeping staff should already know how much chemical to use.
The housekeepers were guessing. The floors of a nursing home were being mopped with a cleaner nobody could verify, in amounts nobody had specified, because the budget had changed and nobody was in charge of making sure it hadn't.
