A second housekeeper, observed two days later, did the same thing, pouring enough to cover the bottom of the bucket. She said she missed the old setup.
Inspectors checked the EPA's registry of certified disinfectants. Medorra Limpreza All Purpose Cleaner Lavender scent was not on it. The bottle itself carried no EPA registration number. When inspectors tried to check the manufacturer's website on multiple dates in January and February, the site returned a critical error. The product could not be verified as hospital-grade by any available means.
The facility's own cleaning policy required EPA-registered hospital disinfectant throughout the building. The facility had no Housekeeping Director. The Regional Nurse Consultant confirmed there was no training for staff on chemical quantities.
The supply breakdown traced back to a management change in September 2025. A central supply employee who had worked in the building for years said things changed drastically after the new management took over. Before, there were never supply problems. After, a budget issue left the facility going from adequate supplies to limited ones. Corporate controlled the ordering. Facility staff had no say in what was approved or purchased for them.
Administrator B told inspectors there were extra supplies in the basement and said staff should educate each other on where to find them. He or she expected housekeeping to already know how to use appropriate chemicals in the right amounts.
The residents living on those floors had no way of knowing what was being used to clean them, or whether it was working.
